Recalls and owner complaints reported to NHTSA

Compared with the typical (middle) count of 131 complaints across the 8 model-year 2016 heavy-duty trucks we cover. Best-selling trucks naturally collect more complaints because more of them are on the road.

What owners should know

Covers 2011–2016 · 6.7L Power Stroke diesel · Updated 2026-10-01

These notes cover the 6.7L Power Stroke diesel. If the truck has the 6.2L gas engine, the engine sections below don't apply, but the front end, rust and transmission notes still do.

We counted 1,053 owner complaints filed with NHTSA for 2011–2016 F-250s with the 6.7L diesel (identified by VIN). Complaint counts show what owners report most often. They don't tell you how many trucks have a problem, and owners with problems are far more likely to file a report than owners without them.

At a glance

  • Front-end shake ("death wobble") is the most frequently reported problem by a wide margin. Check the front end carefully on any 4x4.
  • The high-pressure fuel pump (CP4.2) is the most expensive known failure. Clean fuel and regular fuel filter changes matter.
  • 2011–2012 trucks have extra early-production issues: exhaust temperature sensors, turbochargers and NOx sensors.
  • The transmission (6R140) has a good reputation when it's been serviced and the engine hasn't been aggressively tuned.

Front-end shake ("death wobble")

About 4 in 10 of the diesel complaints we counted (445 of 1,053) describe a violent shaking of the front end and steering wheel, usually set off by a bump at highway speed. It's reported on every year from 2011 to 2016. Owners describe having to brake hard to make it stop.

This isn't unique to Ford. It can happen on any truck with a solid front axle once steering and suspension parts wear. Ford released a service bulletin in 2011 that added a new steering stabilizer, but owners on forums frequently report that a new stabilizer only hides the shake for a while. The fix owners most often report as lasting is replacing worn parts, especially the track bar and its bushings, and checking the ball joints, tie rods and alignment. Bigger tires and lifts without the right correction parts make it more likely.

When buying: have a helper rock the steering wheel with the engine running while you watch the track bar ends and steering linkage for movement. On the test drive, hit some bumps at 50–70 mph. Ask whether the front end has had work and what was replaced.

High-pressure fuel pump (CP4.2)

All 6.7L engines from these years use a Bosch CP4.2 high-pressure fuel pump. When one fails, it can send metal particles through the whole fuel system, and the fix usually means replacing the pump, injectors, fuel lines and rails and cleaning the tank. Owners in NHTSA complaints describe bills of roughly $8,000 to $13,000, often with the engine shutting off while driving.

Failures are commonly linked to fuel problems: water, contamination or poor lubrication. Ford's 2024 CP4 recall (24S78) covers 2020–2022 trucks only, not 2011–2016.

When buying: ask for fuel filter service records and whether the pump has been replaced. Some owners add aftermarket protection (extra filtration, fuel additives, or a "disaster prevention" kit that keeps debris from reaching the injectors). These are signs of an owner who knew about the issue.

2011–2012: early-production issues

The first two years of the 6.7L have more reported problems than later years.

  • Exhaust gas temperature (EGT) sensors. 36 of the 41 EGT complaints we found are from 2011–2012 trucks. Owners describe the engine losing power or shutting down while driving, sometimes more than once. Ford's recall for this sensor covered certain ambulance-type trucks, not regular pickups.
  • Turbocharger. Early turbos used ceramic bearings that frequently failed, often between about 50,000 and 100,000 miles. Ford later switched to an updated turbo with steel bearings. Ask whether the turbo has been replaced. A loud whistle or screech and smoke are warning signs.
  • NOx sensors (2011). Ford ran a program in 2012–2013 to replace these sensors and update the software on 2011 trucks.

None of this means you should avoid a 2011–2012. It means records matter more: a 2011 with a replaced turbo and documented fixes can be a better buy than a neglected 2015.

Emissions system (DEF, DPF, NOx)

Like all modern diesels, these trucks use a diesel particulate filter (DPF) and diesel exhaust fluid (DEF). Complaints include DEF heater failures (code P20BA is frequently mentioned), NOx sensor failures and "exhaust cleaning" (regeneration) running too often. Lots of idling and short trips are hard on these systems.

When buying: look for warning messages about exhaust fluid or exhaust filters. Check how many of the truck's engine hours were spent idling, if the dash shows it. Ask whether the emissions system has been removed ("deleted"). A deleted truck can fail inspections and may be illegal to drive where you live.

Cooling system leaks

Radiator leaks come up often on owner forums, especially on 2011–2013 trucks, sometimes under 100,000 miles. Other frequently reported leak points are the O-ring at the upper radiator hose fitting and the plastic coolant bottle (degas bottle) and its hoses.

When buying: look for dried coolant crust on the radiator, hoses and degas bottle. Check that the coolant level is correct and the coolant is clean.

Rust and cab mounts

In salt-belt states, check the cab corners, rocker panels and wheel arches, and the lower radiator support. A smaller number of owners report rusted cab mounts that let the cab shift on the frame. That's a structural problem worth having a mechanic look at.

Transmission (6R140)

The 6-speed automatic is generally well regarded. Reported problems include harsh or slipping shifts and torque converter shudder, most often on trucks that tow heavy loads, missed fluid changes, or have aggressive engine tunes.

Maintenance tips for owners

These are Ford's intervals as published for these years. Use the "severe service" numbers if you tow, plow, idle a lot or drive off-road.

Service Normal Severe
Engine oil 10,000 miles or as shown on the dash 5,000 miles
Fuel filters (both) 22,500 miles 15,000 miles / 6 months
Transmission fluid 150,000 miles 50,000 miles
Coolant (first change) 105,000 miles / 6 years 60,000 miles
Front axle and transfer case 150,000 miles 50,000–60,000 miles
  • Drain the water separator whenever the dash says "water in fuel." Don't ignore it, because water is hard on the high-pressure fuel pump.
  • Buy fuel from busy stations and replace fuel filters with quality parts.
  • Check the front end (track bar, ball joints, tie rods) at every tire rotation, especially if the truck is lifted or on bigger tires.
  • Avoid long idling when you can. It's hard on the DPF and DEF systems.

Before you buy

  • Have a Ford dealer pull the truck's service and warranty history using the VIN. Owners on forums call this an "OASIS report."
  • Check open recalls at nhtsa.gov/recalls with the VIN.
  • Get a pre-purchase inspection from a diesel shop.
  • Bring our used truck inspection checklist.

Common failures & how to fix them

Parts and video buttons search for your 2016 Ford F-250 Super Duty. Always confirm a part fits using your VIN. Brakes, steering, fuel and airbag work affects your safety, so if you're not sure, have a qualified mechanic do it.

  • Front-end shake (death wobble)

    Moderate DIY

Signs: Violent shaking of the steering wheel and front end after hitting a bump, usually at 45+ mph. Slowing down hard is often the only way to stop it.

Fix: Find and replace the worn parts, most often the track bar or its bushings, then check ball joints, tie rod ends, the drag link and alignment. A new steering stabilizer alone usually only hides the problem.

  • High-pressure fuel pump (CP4) failure

    Easy DIY

Signs: Sudden stalling or no-start, low fuel pressure codes, metal in the fuel filter. Often there's little warning.

Fix: Prevention is the fix you can do: change both fuel filters on schedule, drain the water separator, and buy fuel from busy stations. Some owners add a kit that keeps pump debris away from the injectors. A failed pump usually means a full fuel system replacement at a diesel shop.

  • 6.7L exhaust temperature (EGT) sensor failure

    Moderate DIY

Signs: Check engine light, reduced power, or the engine shutting down. Most common on 2011–2012 trucks.

Fix: Scan for codes to find which sensor failed and replace it. There are four on the exhaust.

  • 6.7L coolant leaks

    Moderate DIY

Signs: Low coolant, a sweet smell, dried coolant crust on the upper radiator hose fitting, radiator or coolant bottle.

Fix: Find the source. The upper radiator hose O-ring is a common and simple fix. Radiators and coolant bottles are bigger jobs.

  • 6.7L DEF heater and NOx sensors

    Moderate DIY

Signs: DEF or exhaust fluid warnings, reduced power, check engine light.

Fix: Scan for codes. Common fixes are a NOx sensor or the DEF heater and pump assembly.

My 2016 Ford F250 was flagged under safety recall notice 25S89, NHTSA recall 25V572 for the issue of where the backup camera could display a blank/distorted image, or message that the rear view camera is unavailable. The camera on my F250 has not completely failed, but has exhibited the known issues mentioned in the recall intermittently. I received the recall notice from Ford and brought my F250 to Wiscasset Ford for a scheduled appointment on 5/9/2026 to address. Wiscasset Ford did not repair my camera or replace any parts based on what they claimed was "a lack of recall codes were present". The dealership afterward must have entered in the database that they have addressed the recall as my VIN now shows no open recalls on it, despite the fact they didn't replace/repair anything. I received a letter from Ford referring to "Customer Satisfaction Program 25N15" stating not to return to the dealer for this repair "UNLESS" if the rear view camera displays a blank or distorted image. On 8/10/2026, my truck exhibited the distorted image/"camera not available" message again on an occasion in reverse. I took photos/video as proof. I once again made an appointment for 9/10/2026 at 8:15AM to address the issue. Upon arrival and explaining the reason for my service appt. to the advisor, I was asked if I was willing to pay for the diagnostic to be done on my F250. I stated no as my issue was based on a known recall and I showed my photos/video of the failure as proof. The advisor excused himself to speak with the shop foreman. Returned saying they'd look at my truck "as a good faith issue", (generous of them...) but they'd have to repeat the "test for codes" process and if the codes were still not there, there was nothing they could do, despite my evidence my truck having the symptoms relative to the recall. Plus I'd have to leave my truck with them all day, despite having an appt. Took 1 hour to test on 5/9. Left as I felt they do not take this safety concern seriously.

Ford has issued a number of recalls for issues on Ford F-Super Duty (F-250) with rearview cameras in their vehicles. I am also experiencing the same issues with my 2016 Ford F-Super Duty (F-250) rearview camera that displays a blank or distorted image causing no visibility while backing up, increasing my risk of a crash. The issue, Covington Ford Inc., Service Department located at 9101 Dr Martin Luther King Ave, Covington, GA 30014 (770-786-3432) explained to me is that it is caused by a loss of electrical conductivity due to fretting corrosion on the terminal header connector inside the camera and misalignment of the connectors. I am also experiencing a full black image on the SYNC screen when placed in reverse or when the 360-degree view is selected as well. Diagnostics were completed 5/21/2025 costing me $150.00. Proof of diagnostics cost can be provided upon request. Covington Ford Inc. indicated I would need a new camera installed at a charge of $727.24 + tax not including the cost of the diagnostics. Estimated total cost of repairs including diagnostic charge would be $877.24 + tax. In addition to being a safety hazard, I use this vehicle to back up to and connect to trailers necessary for my income and need to have this repair completed.

After we drove over a bumpy section of road the truck experienced uncontrollable shaking. The road conditions were not severe, but were the cause of the shaking. During the immediate aftermath no input was possible apart from taking my foot off the accelerator. The shaking was severe and uncontrolled causing other vehicles in the vicinity to react. It felt like the truck was going to flip over. This has now happened several times since we purchased the truck. We recently had new brakes installed, but that is not the reason for the shaking.

On September 3, 2023, the fuel pump failed while the truck was in the corner of an on-ramp for an interstate. The truck immediately lost power while in the corner causing a loss of control of steering. This resulted in a near collision with a concrete dividing wall while attempting to maintain control of the vehicle until the vehicle came to a stop. There were no warnings, check engine lights, or other issues with the truck prior to the incident. The truck was inspected by a diesel repair company where it was determined that the Bosch CP4 fuel pump failed resulting in a catastrophic fuel system failure with metal shavings contaminating the complete fuel system. This required a complete fuel system replacement.

At the exact moment the message displayed, the truck immediately died at 70 mph leaving me to be forced to wrestle the steering wheel to maneuver the truck to the side of the interstate as I had no power steering due to the failure. I have been informed that my truck is a victim of a catastrophic engine failure as a result of a failed CP4 OEM Bosch high-pressure fuel pump. The failure has resulted in the complete contamination of the fuel system requiring a complete overhaul with parts cost at $6,600.00 and approximately $4,000.00 in labor for a total of more than $10,600.00. on a low mileage diesel.

The failure of the APIM has completely disabled the Rearview Backup Camera, which is a federally mandated safety feature (FMVSS No. 111). Operating a heavy-duty F-250 without a functional backup camera creates a significant blind-spot hazard and an immediate safety risk to pedestrians and property. King Ford has confirmed the failure. We called our local Ford dealership, Matthew Ford of Pryor (Oklahoma) and made an appointment for the safety recall of replacing the clock spring. They proceeded to attempt the replacement and in the process damaged the steering column module, part no. GC3Z3F791KA. We are now being told that Ford does not have the necessary steering column module part and nobody manufactures it for our trim level, Platinum. Consequently, our truck is not able to be fixed and returned to us in the same condition it was given to the dealership. It is still at the dealership, in pieces. We dropped it off on March 27th, 2025.

I was driving my truck on a highway in [XXX]. I was going 70 MPH when we crossed over a bridge. The pavement was lower than the bridge cement. that cause the truck to have a slight bump when crossing over it. That cause the entire truck to begin a violent shaking (death wobble). it was very hard to control the truck. I had to step on the brakes very hard to slow down almost completely on the highway to stop the shaking. all the other traffic was trying to avoid my truck. fortunately no accidents occurred. I've spent $4000. dollars to try to fix the problem. Every time the Ford dealership says there is nothing else they can do. How long is it going to take for you to force Ford to fix or Lemon law these trucks? INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

The problem is commonly called "DEATH WOBBLE". The steering system enters an extremely violent wobble/shake making control of the vehicle nearly impossible. 2 independent service centers have addressed the issue. The first did repairs, including warranty replacement of a faulty part and the truck still has the "DEATH WOBBLE". The 2nd independent service center made note of the 2nd faulty replacement part (TRACK BAR) and reports "TRACK BAR BUSHING IS MOVING EXCESSIVELY. TRACK BAR SHOULD BE REPLACED." No other physical inspections have been performed. There were no vehicle warning lights due to this issue. There were a few incidences of the "DEATH WOBBLE" prior to my first appointment with an independent service center. That first appointment was approx. 2/19/2024, completed on 2/27/2024. Second time completed on 6/21/2024.

On [XXX], I was driving westbound on [XXX], between Kerrville and Junction, Texas. I noticed a diesel fuel smell and pulled off the highway to investigate the source. Upon stopping, fire erupted from the engine compartment and quickly spread to the front dashboard and front passenger seating area. No warning lamps, no high temp or abnormal gage levels. The Mountain Home, TX volunteer fire dept arrived about 30 minutes after we called 911 and extinguished the remaining flames. They commented that a cracked / failed secondary fuel filter housing could be to blame. My insurer, State Farm, under claim #[XXX], now has possession of the vehicle remains. I told State Farm that NHTSA opened an investigation into possible fuel filter housing failures / fires. I do not know what their intentions are with the vehicle remains. I was attempting to access the interstate when my truck began to immediately shut down at 70 miles per hour. I received a warning as the engine shut down stating that my engine power was reduced.

Vehicle has 44,000 miles, has never been wrecked in any way. Recently the advance trac system began displaying a fault at random. The light would come and go. At the time the fault light was active the turn signals did not operate normally. A message noted to service advance trac system. ( which is related to the abs) I noticed, once, the abs activate momentarily for no apparent reason. I narrowed the fault down to the steering wheel angle sensor. I had a Ford service center confirm the issue was the steering angle sensor. The sensor was on a backorder. I called around and other dealers confirmed. A week later while entering a cloverleaf and slowing down the system faulted, illuminated the error, and the abs activated. The brake system applied brakes unevenly pulling the vehicle to the left then right and aggressively slowing down without driver brake input. I had to fight to control the vehicle to prevent losing control or get rear ended. The roads were clean, dry, and it was daylight. Upon that i could see the complete loss of steering wheel angle sensor input. The dash can show this readout. Without the steering wheel angle input the system, which is to prevent accidents, nearly caused one. I tried turning off the advance trac via the traction control button, this was ineffective. I had to remove the abs pump fuse to get home safely. This system should not in any way be able to cause this severe loss of vehicle control simply due to the loss of one sensor. This is absolute unsafe software protocall. Attatched are images of the sensor reading then no place holder for it when it does not read.

HAS A HISSING SOUND UNDER THE PASSENGER SIDE ,HAS BEEN GIVING US UPSET STOMACHS AND SOMETIMES HEAD ACHES,JUST PLAIN SICK FEELING, WHEN THE AC AND DEFROST ARE ON. ALSO FROM END ABS SEEMS TO ENGAGE WHEN GOING AROUND A BEND AND THEN SUPPOSEDLY THE AUTO CORRECT KICKS IN AND TRIES TO CORRECT THE TRUCK PULLING STEERING WHEEL. WAS TOLD THEY TESTED ONE TRUCK AND IT DID IT ONE TIME,THIS IS AFTER THEY PUT IN COUNTLESS HRS ON LABOR PLUS A $1000 INTO A NEW STEERING COLUMN AND DETERMINED THAT IT IS NORMAL OPERATING CONDITION. DIDN'T BUY IT FOR THIS GOD FOR SAKE NOISE . AS FOR THE HISSING,TOLD ME TO DETAIL THE TRUCK AND THAT THEY WERE NOT GOING TO DO ANYMORE FOR THIS. THIS HAS BEEN GOING ON SINCE THE FIRST WEEK OF BUYING THIS TRUCK. OVER AND OVER TO THE DEALERSHIP,THEY KEEP PUTTING NEW PARTS ON. IF THIS IS NORMAL OPERATING CONDITIONS,THAN WHY DON'T THEY KNOW WHAT TO REPLACE AND KEEP PUTTING MORE TEST AND PARTS. NO ANSWER ON THE HISSING SOUND AND VERY VERY RUDE MANAGERS AT DEALERSHIP.

When I drove over 50mph and hit any bump the front of the truck shakes wildly. Online says DEATH WOBBLE. This has been going on for 4 months. When taking my children to school 20+ miles away on the highway this has happened several times to and from school on the same day. Other vehicles move out of my line to avoid me while I slow the truck to under 45 mph. They are scared and nervous about getting to school safely. My mother and wife refuse to drive the truck after experiencing this while driving. The fuel injector now also has a problem. The truck is going in to be serviced for the failing fuel injector and death wobble hopefully today if Waldorf MD Ford has openings I am awaiting a call back. It has not been inspected yet only diagnostic at AutoZone showing the fuel injector error code. No warning was given prior to this happening, the truck was riding smoothly, hit a bump in the highway and went crazy. Then it did it again and again and again.

DRIVING ON INTERSTATE AND THE TRUCK STARTED A UNCONTROLLABLE WOBBLE. DURING THE WOBBLE I FELT I HAD NO CONTROL OF VEHICLE AND WAS MOVING OUT OF MY LANE. FORD NEEDS TO DO SOMETHING IMMEDIATELY BEFORE SOMEONE GETS KILLED I ALSO TELL A FIFTH WHEEL CAMPER AND I CAN ONLY IMAGINE HAVING THAT KIND OF WEIGHT BEHIND ME THAT THE TRUCK WOULD FLIP AND POSSIBLY KILL ME OR SOMEONE ELSE ON THE INTERSTATE.

12/07/16 I PURCHASED A BRAND NEW VEHICLE. I DROVE IT HOME APPROXIMATELY TWO MILES TO PICK MY WIFE UP AND TAKE HER FOR A SPIN. UPON STARTING THE VEHICLE THE AIR BAG LIGHT, THE STABILITY CONTROL LIGHT AS WELL AS THE WARNING MESSAGE FOR SERVICE ADVANCE TRAC AND TRAILER BRAKE MODULE FAULT AND THE 911 ASSIST NON OPERATIONAL. I TOOK THE TRUCK BACK TO THE DEALERSHIP AND THEY ENDED UP REPLACING THE RESTRAINT CONTROL MODULE. THEY HAD THE TRUCK FOR ABOUT FIVE DAYS. WHEN I PICKED UP MY TRUCK BEFORE I EVEN GOT OFF THE LOT ALL THE LIGHTS AND MESSAGES CAME ON AGAIN. I TOOK IT TO THE DEALERSHIPS OTHER LOCATION AND THEY HAD IT ROUGHLY THREE WEEKS. THEY REPLACED THE INSTRUMENT CLUSTER AND THE MASTER WIRING HARNESS AT THE INSTRUCTIONS OF FORD ENGINEERING. I DROVE THE TRUCK BEFORE HAVING ALL THE SAME THINGS HAPPEN AGAIN WITH THE EXCEPTION OF THE STABILITY CONTROL/ SERVICE ADVANCE TRAC MESSAGE. I TOOK THE TRUCK BACK AGAIN , THIS TIME THEY REPLACED THE RESTRAINT CONTROL MODULE AGAIN. A FEW WEEKS WENT BUY AND NOW THE SAME PROBLEMS AGAIN. THIS IS THE FOURTH VISIT AND TOMORROW MARKS THE 30TH BUSINESS DAY THE TRUCK HAS BEEN IN THE SHOP. ADD TEN MORE DAYS FOR THE WEEKENDS IT SAT THERE AND THE ONE DAY FOR WHEN THE DEALERSHIP HAD TO REPAIR MY LEATHER INTERIOR THAT THEY MESSED UP. THAT'S 41 DAYS AND I HAVEN'T EVEN HAD IT FOUR MONTHS YET. I'VE REQUESTED THAT THE MANUFACTURER BUY IT BACK NOW TWICE AND WAS DENIED BOTH TIMES. I WAS TOLD THAT MY VEHICLE DOES NOT MEET THEIR CRITERIA. THE SERVICE DEPARTMENT TOLD ME THAT THE VEHICLE IS UNSAFE TO DRIVE WHEN THESE LIGHTS COME ON. IT POWERS DOWN THOSE SYSTEMS MAKING THEM NON OPERATIONAL. THERE IS NO PATTERN TO THIS HAPPENING. IT CAN BE AT START UP ,SITING STILL OR MOVING DOWN THE ROAD. THE LIGHT CAN STAY ON THE ENTIRE TRIP OR JUST FLASH MOMENTARILY.

While driving the morning of 13 Jan 2025 my Airbag sensor light on the dashboard illuminated. Shortly after that me and my wife smelled smoke in the vehicle cabin and then we heard a loud explosion and the seatbelt instantly tightened around my chest and waist. It was after I was able to stop safely and research the incident that I realized my drivers side “pretensioner” detonated while I was driving.

The contact owns a 2016 Ford F-250. The contact stated that while driving at approximately 50 MPH, the check engine light illuminated, and the message “Reduced Power Mode” was displayed. The vehicle was taken to a dealer where it was diagnosed that the fuel pump, manifold assembly, fuel injectors, injector terminal seal, gaskets, vacuum pump, and left side fuel rail pressure pump needed to be replaced. The manufacturer was notified of the failure. The failure mileage was approximately 76,880.
