A software error may cause the rearview camera image to be obstructed by the infotainment home screen menu when the vehicle is placed in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
Risk: An obstructed rearview camera image can reduce the driver's rear visibility, increasing the risk of a crash.
Fix: The accessory protocol interface module (APIM) software will be updated by a dealer or through an over-the-air (OTA) update, free of charge. When towing a trailer, the integrated trailer module (ITRM) may lose communication with the vehicle, possibly causing a loss of brake and turn signal lights, or a loss of brake function. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 108, " Lamps, Reflective Devices, and Associated Equipment."
Risk: Trailer lights or brakes that do not function can reduce the driver’s ability to control the trailer and decrease the trailers visibility to other road users, increasing the risk of a crash.

Risk: A misrouted seatbelt may not adequately restrain an occupant in a crash, increasing the risk of injury.

Ford Motor Company (Ford) is recalling certain 2021 F-150 vehicles. The steering gear may have internal damage, which could result in difficulty steering, or the inability to rotate the steering wheel while driving.
Risk: Loss of steering control can increase the risk of a crash.
Fix: Owners are advised to not drive their vehicles until the repair is complete. Vehicles will be towed in for service, and dealers will remove and replace the steering gear, free of charge. Ford Motor Company (Ford) is recalling certain 2021 F-150 and 2020-2021 F-250, F-350, F-450, and F-550 Super Duty vehicles. During manufacturing, the windshield may not have been properly bonded to the vehicle, allowing it to detach in a crash. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 212, "Windshield Mounting."
Risk: In the event of a crash, if the windshield separates from the vehicle, it can increase the risk of injury.
Fix: Ford will notify owners, and dealers will remove and reinstall the windshield, free of charge. The recall began April 16, 2021. Owners may contact Ford customer service at [phone removed]. Ford's number for this recall is 21C06.

The contact owns a 2021 Ford F-150. The contact stated that while driving at various speeds, the transmission unexpectedly downshift. Additionally, the vehicle hesitated and jerked while driving. The vehicle was taken to the dealer on several occasions; however, the dealer failed to duplicate the failure. The failure persisted and the dealer disassembled the transmission and replaced the hub assembly, Part Number: ML3Z7H351B; however, the failure persisted. The vehicle was returned to the dealer where a service technician test drove the vehicle with the contact and the technician stated that the transmission needed to be replaced. After months of waiting for a response, the contact took the vehicle to another dealer for assistance. The second dealer test drove the vehicle and disassembled the transmission and recommended that there was an adjustment of the clutch clearance, replace the necessary seals, and replace the C-clutch and seal. The technician acknowledged the failure and provided the recommendation, and the contact was later informed that the dealer would only clear the codes. The technician suggested that the contact monitor the vehicle for future issues. The contact stated that the failure immediately returned after the codes were reset. The failure mileage was 19,605.

Experiencing total stop of acceleration in two occasions: 1) when putting the car in reverse and pressing the accelerator and 2) when making a left or right turn after a yield or stop. And when I say loss of acceleration, I don’t mean hesitation, reduced power, or sluggish acceleration. I mean absolutely ZERO acceleration. I’ll press the accelerator and nothing happens. The engine stays basically at idle, and the truck will not accelerate for roughly 5-10 seconds, regardless of how much accelerator I’m giving it. Then suddenly power comes back and the truck drives normally again. This happens with normal operating temps while driving most of the day. It got to a point where it happened once every day. Very scary stuff when you're pulling out into traffic - major safety concerns with oncoming traffic.

I am reporting a severe safety defect where Ford Motor Company's sequential safety recall software updates are actively corrupting powertrain modules and inducing physical mechanical failures while the vehicle is in operation.Following the completion of the 24S55 Engine Intake Valve Recall (which reflashed the PCM and wiped the transmission's adaptive learning tables), my 2021 Ford F-150 developed an immediate operational shudder. Shortly after, a forced Over-the-Air (OTA) update for the 26C10 Trailer Brake Module triggered a secondary network reset via the CAN-bus. The intersection of these two software-driven actions caused an operational fault that physically broke the torque converter clutch.A compromised torque converter clutch can cause sudden vehicle stalling, unpredictable acceleration lags, and catastrophic powertrain locking while driving at highway speeds, creating an imminent crash hazard. The local Ford dealership service department confirmed the hardware failure and noted that pushing automated software updates over unstable tables leaves the entire transmission highly vulnerable to total systemic failure.Ford is currently refusing to open an escalation file or address this issue. Because Ford's official remedy for safety recalls 24S55 and 26C10 directly introduces a secondary, severe safety risk to the vehicle's powertrain integrity, I request that NHTSA investigate the technical compatibility and safety of these sequential software updates.

This vehicle was purchased new. It currently has about 56000 miles. Over time there have been numerous incidents where the battery was suddenly dead. The dealer repeatedly replaced the battery with out addressing the cause. This was done under warranty until recently when there was a charge of $1138. This supposedly resulted in the electronics being reset to factory settings. On a subsequent trip of about 2000 miles , there were numerous incidents (at least 8-10) where the Google Maps program, installed by the dealership, malfunctioned. My question is, what other electronic issues were installed with the factory settings. This has been reported to the dealer, with no response. On August 31, 2026, a request was made to the dealer for copies of service records for this vehicle so that this problem and lack of correction could be challenged. There has been no response, other than their saying “they will make it right “. They are apparently unable to provide service records.

Due to a dangerous design flaw in the 2021 Ford F150, I was put in a potentially dangerous situation by completely losing all power in the vehicle on a backroad. Design flaw is the DCN module, which floods into the vehicles electrical system/body of vehicle through the cowel drain that can supposedly be blocked by a singular piece of pine straw. Due to the design flaw, the vehicle completely lost all power and all electronics failed while driving down the road at 45 mph, putting both me and my father (riding passenger seat) in immediate danger. Ford knows and is aware of the design flaw and has refused to issue a recall, not only affecting my safety, but now I am going back and forth with ford over the fix to the tune of $2800 that they refuse to fix (vehicle is a gold certified cpo, has extended warranty). When asked how to prevent in the future, ford was and is not aware of how to prevent in the future, and I can not get to the cowel drain clog without professional equipment and ford performing. There is no way to tell the cowel drain is clogged or not, and there is not anything in the ford owners manual related to maintenance of the cowel drain. Again, due to the inappropriately designed drain, this caused water leakage into my truck, posing an immediate threat to my life and others around me. I have all of the videos and pictures needed to show the failure of the system to provide to move this forward to get resolved. I had complete loss of power and electronics on the road. I tried to attach video but it would not let me. I can attach via email when requested.

When backing up, the rear camera display shows a gray filter over the rear image on the display cluster and the right side warning sensors indicate a blue trapezoid on the cluster for the right side of the vehicle. The driver dashboard also has an alert that the rear brake assist feature is inactive

ON YOUR VEHICLE, THE WHEEL END HUB BOLT IN THE REAR AXLE MAY BECOME LOOSE OR BREAK. CONTINUED DRIVING WITH A BROKEN BOLT MAY LEAD TO WEAR WITHIN THE AXLE AND DAMAGE THE AXLE HUB SPLINES. I’ve been contacting ford dealerships in my area trying to get this safety recall replaced over the last year every two or three months. I get the same excuse the part is on back order, we have 70,000 on back order others have said 35,000. Yet there is no timetable for when the part will be available. In the meantime I have to drive a vehicle that has had the top of the bolt connecting the tire to the axel snapped off. Ford has completely failed in remedying this issue since the recall and by their numbers making it so 70,000 people possibly have a broken vehicle that they are not repairing or replacing like the safety recall states.

Trailer brake controller installed in truck by manufacturer. We wanted to tow our travel trailer and was doing the setup process to test before we left for a trip. We were unable to set the gain for the trailer brake controller. Trailer was connected to truck and plugged in, but unable to change gain or feel brakes when we took a drive around the neighborhood. If we had not taken the steps to test prior to heading out on our trip could have been a serious result. Contacted our local Ford dealer and made an appointment for July 8, 2026. Was told the trailer brake controller connected in the back was not connected. They said this could have happened during manufacturering or just fell out while driving. Still either way could result in serious issues. Not sure if my truck was the only one or are there others on the roads with an issue. If an invoice was provided by the dealership, I have misplaced. They wanted to charge me almost a 100 for them to find it was not connected. The split the cost with me.

Hi, I have been trying to get Ford to address the recall 23V896 for over a year and they have been claiming that they dont have parts. They keep providing a different day that they are supposed to receive the parts but when the date comes they provide a different time. I need your assistance with getting this recall implemented.

Within the last 6 months, the driver's side front door, the driver's side rear passenger door, and the passenger side rear doors actuators stopped working and the doors do not lock. The doors do not lock manually with the key fob or lock automatically when the truck reaches 13 mph. This is a safety issue as passengers could inadvertently open the doors when driving when the doors should be locked. This also prevents the factory installed alarm to work properly. it seemed odd that the 3 actuators seem to stop working around the same time.

Should be able to simply repair my daytime running lamps at a reasonable cost. One of them is now dim, causing lower visibility for other drivers. Ford's cost for me to repair was quoted today at over $2,100. Simply put, unreasonable! This is an issue that has effected several vehicles.

Daytime Running Light in the headlight assembly overheated and stopped working. Thousands of 2021-2023 F150 owners have had the same issue. This is clearly a mass scale manufacturer known defect and Ford already tried sending an update to lower the LED brightness but the issue still happens.

Led headlight drl strip turned blue on one side. Have seen many with this issue in the led module ballast. Safety concern because its a common running light issue that hinders people from seeing the vehicle as easily.

On September 10, 2026 about 9:10 am, I experienced an engine stall as I was approaching an intersection. I was slowing down and entering a left turn lane. Vehicle bucked like a horse and stalled. No warning lights or codes. It costed to a stop at which time I attempted a re start. The motor turned over for about 10 seconds and then started up. It seemed like there was a loss of fuel. This incident reminded me of a recall letter a got in the mail last year. After research I saw recall #25V455 with no fix yet. Called my local dealer and they said there is no codes so they would probably have to charge me a diagnostics fee of $250.00 and nothing they can do about the recall. Made an appointment and then I cancelled it. What is a person supposed to do? Drive until something bad happens? Take chances and pray for the best?

The substance obstructed the vision while driving and worsened while driving at night. Despite the independent mechanics best efforts, the film-like substance remained. ABS Control module and brake booster assembly failed. Vehicle would not crank or start. Disconnected battery negative pole for one minute to reset PCM. Vehicle started. Drove immediately to independent service center without difficulty. Service center confirmed vehicle no start the next morning. Found several DTC codes including U3001 (Control Module Improper Shutdown) U300A (Ignition switch) U0121 (Lost communication with ABS) U0554 (Invalid Data recieved from Accessory Protocol Interface) and U0415 (Invalid Data Received from ABS) Flowchart determined ABS module is bad. Anti Lock Brake System control module failed which rendered the system inoperative. Warning on dash that Anti Lock Brake System failed was prominent among other warnings. (Traction Control, Service Needed, Door Ajar (was closed), Hood ajar (was closed) Assembly was replaced and ABS module reprogrammed by independent service center. They retained the bad assembly.

THIS TRUCK THAT I HAVE PURCHASED FROM PALM COAST FORD HAS A RECALL EVERY MONTH THAT KEEPS ME FROM UTILIZING THE VEHICLE. THE LATEST RECALL THAT THERE IS NOT A FIX FOR YET, I WAS TOLD IS A COMPLETE RISK AND I SHOULD NOT EVEN DRIVE THE TRUCK. I REQUEST THAT YOU LOOK INTO ALL OF THE RECALLS THAT HAVE BEEN LISTED ON THIS PARTICULAR VEHICLE AND KNOW THAT I AM TAKING THIS UP WITH FORD, BUT ALSO WANTED TO INFORM YOUR DATABASE OF THE ISSUES AS WELL. I HAVE ONLY HAD THE TRUCK FOR ABOUT A YEAR AND IT HAS BEEN RECALL AFTER RECALL AND THIS ONE THERE IS NOT EVEN A FIX FOR IT TILL NEXT YEAR, AND I AM NOT SUPPOSED TO EVEN DRIVE IT CAUSE IT IS HAZARDOUS.

My 2021 Ford F-150 intermittently displays the warning message “Reverse Brake Assist Not Available – See Manual” when the vehicle is placed in reverse. When this message appears, the reverse brake assist / parking collision warning feature is not available, which may reduce safety while backing up. The vehicle was inspected by a Ford dealership. The dealership confirmed DTC U3000:49 in the IPMA module and identified Ford TSB 23-2040 as applicable to the concern. The dealer stated that the repair requires an IPMA software/module update and calibration. The quoted repair cost was approximately $1,100. This appears to be a software/programming-related condition affecting a driver assistance and brake-related safety feature, not normal wear and tear or a failed mechanical component. The warning is intermittent, but it has appeared multiple times. The vehicle is available for inspection upon request. No crash, injury, or fire has occurred. Ford Customer Support was contacted and case number XXX was opened. Ford advised that no active warranty, recall, or customer satisfaction program currently provides repair cost coverage, and that because this involves Reverse Brake Assist, I may choose to file a safety concern report with NHTSA. When I'm driving my truck, usually when I have come to a stop on an incline, the parking/emergency brake will engage on its own. For example, most recently I was pulling out of a driveway with an incline. I was stopped at the bottom of the driveway to wait for the roadway to clear before I pulled out. When I hit the gas to go, the rear wheels skidded and the truck came to a stop in the oncoming traffic lane. I looked at the dashboard and saw that the parking brake symbol was on, indicating that the parking brake had engaged. I did not engage the parking brake- the truck did it on it's own.

Fuel pump failed while driving causing vehicle to be disabled in a roadway blocking traffic. The vehicle was then incapable of shifting to neutral so that it could be moved out of the roadway. A Ford dealer confirmed the faulty fuel pump and replaced the part. Ford appears to know this is a safety issue as per recall campaign 25S75/25V455 but they have refused to pay for the replacement fuel pump.

My husband was driving on the highway with my two daughters in the car when the vehicle stopped driving and he was barely able to pull off the road. It appears to be a defective low-pressure fuel pump. This is an active recall for this model and year, however Ford says that our specific VIN was not involved in the recall. My family could have been killed in this incident!

When using the window wipers, they become inoperable. Stopping in the middle of the window, pausing when moving, or completely stopping and unable to get them to start moving until they begin to move on their own. This has occurred multiple times and at multiple different speeds of operation for the wipers. This was a previous recall issued by Ford that has been completed on the vehicle yet the same result is occurring.

The rear window basically has 3 parts. Driver side, center sliding section, and passenger side. Only the driver side section defrost. According to online forums specifically for Ford trucks, this is a common issue. Ford’s Special Service Message (SSM) 48490 does reference this as a known issue.

Here is what happened on Feb 21st 2026 at about 8:00 AM MST. We were driving east in the mountains on I-70 in Colorado approaching the Eisenhower tunnel. The truck had been parked in a heated garage overnight. There had been some snow the night before which had been melted by sand and salt, so the road was wet and messy. The vehicles ahead and around my truck were kicking up a mist of water and sand and dirt. My windshield quickly became covered and obstructed. I attempted to apply some windshield washer fluid, but nothing came out. To make matters worse, the wipers automatically turned on turning the speckled dirt and sand to be smeared across the windshield making it almost impossible to see. The smear was too dry that any further wiper action did nothing to clear the windshield. This happened while entering the tunnel. In the dark or the tunnel I could barely see the road – only by ducking way low and peeking through a small portion of the windshield below the wiper smear. There is no place to pull over inside the tunnel. As we exited the tunnel, we were facing the east sun which lit up the smear and made the windshield completely obscured - even my last tiny piece. We were still at 50 MPH with surrounding traffic. My only option was to ease right and hope the emergency lane was clear. I had to hang my head out the side window to determine how far to ease right. It was a miracle we were able to pull off and not hit any other vehicles and come to a stop (I was completely blind). I tried the washer fluid many more times but nothing. I opened the hood and verified I had plenty of fluid and that it was liquid and not frozen. I checked the spray outlets and made sure they were not blocked in any way. Nothing. The washer spray would not work. After turning the truck off and back on - the washer/wipers worked fine. I believe this is a software issue. My family and I were quite shaken up.

The windshield wipers on my 2021 Ford F-150 are inoperable. The wipers suddenly stopped working with no warning. The vehicle had already received Recall 23S10, which was supposed to replace the defective wiper motor arm. However, the wipers have failed again, suggesting the recall remedy was inadequate or that defective components are still being used. Ford and the servicing dealer refuse to reopen the recall or provide coverage, even though this is clearly the same safety issue. Without functioning wipers, the vehicle is unsafe to drive in rain or snow. I request that NHTSA investigate whether Ford’s recall 23S10 remedy is effective and whether additional corrective action or a new recall should be issued to address continued wiper motor failures on these vehicles.

Took my 2021 Ford F 150 in for recall 23s65 and oil change and was told vehicle was unsafe to drive and even though parts according to ford where available the dealer said they were on back order with no ETA of when they would be available and they would put me in a rental for the time being. I’ve been told 3 times now by Ford parts are available but yet the dealership keeps saying they’re still on back order.

My Ford F-150 is subject to Manufacturer Recall Number23S65/NHTSA Recall Number23V896. I asked for the repair multiple times, but the dealership repeatedly told me that the parts were on backorder. The hub bolt on my right rear tire cracked and the truck was making a high pitched sound. I took it to the dealer and they said the bolt was cracked and driving it in that condition is dangerous. The truck is not safe to drive, but the parts remain on backorder indefinitely.

The manufacturer is unwilling or unable to perform recall 23V896 in a timely manner. I have submitted my truck for inspection at an authorized dealer, Gullo Ford of Conroe, TX, on October 6, 2025 per manufacturer requirements. As of this writing, January 14, 2026 I have not been advised of availability of parts to remedy the defect. I am asking that necessary parts be supplied and the recall be performed in a timely manner, as soon as possible at this point.

Ford has refused, on multiple occasions and at multiple dealerships, to address the NHTSA Recall Number 23V896 (MFG. Recall Number 23S65) issue with the Wheel End Hub Bolt in the rear axle. Well, this weekend, the bolt on the passenger side rear axle broke off, rendering the vehicle undriveable. I brought it in to the Lithia Ford Dealership (who's online information said they were open) to get it addressed. They were closed, no one was there, and they won't even give me a loaner vehicle while the F-150 is inoperable. This recall has been out for two years now and they still won't deal with it. Absolute neglect of their obligations and an unsafe practice!

Vehicle Information 2021 Ford F-150 XL Supercrew approximately 35,000 miles. While traveling southbound on [XXX] near [XXX], Ok, I was operating my 2021 Ford F-150 XL at approximately 75 mph with the cruise control engaged. As I approached slower traffic ahead, I attempted to disengage the cruise control by pressing the cruise control cancel/off button on the steering wheel. Despite pressing the button multiple times, the cruise control would not disengage. Same thing while tapping the brakes it would not disengage. The vehicle continued to maintain speed, and the cruise control system remained unresponsive to my commands. In response, I applied the brakes to safely reduce speed, I did not perform a panic stop or abrupt braking maneuver. Once I had the vehicle slowed, I moved safely onto the shoulder of the highway, well away from traffic. I then shifted the vehicle into park, shut off the engine, and restarted the truck. After restarting the vehicle, I elected not to use the cruise control system for the remainder of my trip, which lasted approximately eight additional hours to Houston, Texas, I have not used the cruise control since the incident because I am concerned that the condition could recur and create a safety hazard. No collision, property damage, or injuries occurred. However, the apparent failure of the cruise control system to disengage when commanded raises significant safety concerns and warrants investigation. Ive noticed that the electric motor sometimes doesnt want to disengage quickly when I step on the brakes. In this incident I had to make a hard brake because someone pulled out in front of me. I had the cruise control on. When I slammed on the brakes the vehicle did not stop fast the vehicle felt as if it was still being pushed by something. Then a small jolt was felt and then the vehicle finally slowed. Had I been going faster or had the car that pulled been closer. This fault would have resulted in an incident

Subject: Steering Rack Failure Reimbursement Request, VIN [XXX] To Whom It May Concern, I am requesting review and reimbursement for an electronic steering rack replacement on my 2021 Ford truck, VIN [XXX]. The electronic steering fault first appeared at start-up and when turning the wheel to full lock. Initially, I could pull over, shut the truck off, restart it, and the warning would clear. After a battery replacement, battery monitoring system reset, and multiple steering relearns, the steering failed completely. When the failure occurred, steering became extremely difficult. After hitting a bump, the steering wheel jerked suddenly and caused pain to my hand and thumb. This also created a serious safety risk for me and others on the road due to the reduced ability to steer. The dealership replaced the electronic steering rack at a cost of over 2,000. The truck had only about 40,000 miles at the time. I later found Ford Recall 21S28, which appears to cover certain trucks built between XXX. Based on my VIN and build date, my truck appears to fall within that range, but the dealership advised my VIN was not included. Given the low mileage, safety risk, and apparent build-date match with Recall 21S28, I request that Ford and NHTSA review my vehicle’s eligibility under this recall or any related program and reimburse me for the repair. I understand there was a recall on some 2021 trucks with similar steering issues, although my VIN is not included. My vehicle was left rendered unable to steer during use. It was taken to a local shop All Japanse Auto, who found the damage was consistent with Ford Saftey Recall 21S28. Upon speaking to AutoFair Ford of Haverhill service they informed me Ford did not include my truck in the recall despite it matching year and model and they would not be addressing the issue for me. Loss of steering mid use is amazingly careless and should be cause for taking more aggressive action

SYSTEMS & COMPONENTS THAT FAILED: A faulty or clogged engine bay drain plug failed to divert water, causing significant water accumulation that routed into the interior cabin. This water intrusion directly soaked interior carpeting and exposed the Data Communication Network (DCN) connector and Body Control Module (BCM) wiring harness to moisture, resulting in severe pin corrosion and electrical communication faults. The vehicle and all affected components are fully available for inspection upon request. SAFETY RISK: The DCN connector and Body Control Module control critical vehicle operations, exterior lighting, instrument clusters, and safety/driver-assistance systems. Water corrosion on these communication buses causes unpredictable, uncommanded electrical failures and module communication dropouts while operating the vehicle at speed, creating a severe crash hazard to occupants and surrounding traffic. DEALER CONFIRMATION & INSPECTIONS: The issue was physically inspected and confirmed by an authorized Ford dealership service department, Kayser Automotive Group located at [address removed]. Good Afternoon. The Pro Power Onboard high-voltage electrical system has experienced a permanent malfunction, locking out with a "Ground Fault Detected" warning. The vehicle is a 2021 Ford F-150 Rocky Ridge conversion. This failure stems directly from a defective or incomplete execution of Safety Recall 23S35 (NHTSA Campaign 23V613000).The initial remedy applied by the dealership consisted only of adding protective tape to the rear wiring harness. However, due to the increased travel and ride height of the official Rocky Ridge suspension lift, the wiring harness pulled out of its factory retaining side clip. This caused excessive tension and stretching on the high-voltage lines, completely exposing the bare interior colored wires and leaving them vulnerable to road moisture and friction against the metal frame.The system now triggers a permanent short-to-ground error, creating an imminent risk of an electrical short circuit or vehicle fire under the high-voltage (120V/240V) lines. The Ford dealership and corporate customer care refuse to remedy this recurring safety hazard under the recall warranty because it was previously marked as completed, even though the applied remedy failed to safely secure the harness. After a day of heavy rain, the vehicle stalled at a stop light and would not restart. The steering wheel and transmission locked, preventing the vehicle from being placed in neutral, creating a safety hazard as the vehicle was immobile in traffic. No warning lamps appeared prior to the failure. The vehicle required a tow. The dealer confirmed water had intruded through the passenger side cowl/firewall seam due to inadequate factory sealing, tracking down and pooling directly onto the Body Control Module (BCM) mounted on the passenger side firewall. The BCM failed due to water damage, causing PATS desynchronization and the no-start condition. The BCM failure subsequently shorted out the starter motor. Dealer replaced the BCM, repaired the leak, and replaced the starter. Total repair cost approximately $3,200. Failed components are available for inspection at the dealership. This identical failure; passenger side cowl water intrusion leading directly to BCM damage — is documented across numerous 2021+ F-150 owner forums, indicating a widespread factory sealing defect at the cowl/firewall/inner fender seam junction.

My 2021 Ford F-150 intermittently displays the warning message “Reverse Brake Assist Not Available – See Manual” when the vehicle is placed in reverse. When this message appears, the reverse brake assist / parking collision warning feature is not available, which may reduce safety while backing up. The vehicle was inspected by a Ford dealership. The dealership confirmed DTC U3000:49 in the IPMA module and identified Ford TSB 23-2040 as applicable to the concern. The dealer stated that the repair requires an IPMA software/module update and calibration. The quoted repair cost was approximately $1,100. On [XXX], I was driving westbound from Missouri into Oklahoma in my 2021 Ford F-150 XL. While traveling approximately 75 mph on the highway, I moved into the passing lane to overtake another vehicle. As I accelerated and applied additional throttle to pass, the transmission downshifted into passing gear. After completing the pass, the vehicle continued to accelerate on its own even after I released the accelerator pedal. The engine remained at a high throttle setting, and the truck increased speed to approximately 85 mph. At that time, I was not using cruise control, nor had I pressed the cruise control switch, which was involved in a previous acceleration-related incident I experienced with this vehicle. To regain control, I carefully created distance from surrounding traffic, applied the brakes, and maneuvered the vehicle onto the shoulder. After coming to a stop well off the roadway, I shifted the vehicle into Park and turned off the engine. I then restarted the truck, and the unintended acceleration condition was no longer present. Because of the incident, I did not attempt to use the cruise control system again during the trip, and I avoided using heavy throttle or passing gear out of concern that the condition could reoccur. The vehicle has since been inspected by a Ford dealership. As of this report, their evaluation has not identified the cause of the problem, and no fault has been found despite the incident occurring on the highway. This incident created a significant safety concern because the vehicle continued to accelerate after the accelerator pedal was released, resulting in an unexpected increase in speed and requiring me to pull off the highway and shut off the vehicle to regain normal operation. My vehicle experienced significant transmission shift issues, skipping gears, stuck in gears, going to wrong gears, and inability to shift, that left me disabled in intersections multiple times. The situation worsened over time and had to have the transmission replaced. The failure was diagnosed as a CDF coupling failure.

Due to a dangerous design flaw in the 2021 Ford F150, I was put in a potentially dangerous situation by completely losing all power in the vehicle on a backroad. Design flaw is the DCN module, which floods into the vehicles electrical system/body of vehicle through the cowel drain that can supposedly be blocked by a singular piece of pine straw. Due to the design flaw, the vehicle completely lost all power and all electronics failed while driving down the road at 45 mph, putting both me and my father (riding passenger seat) in immediate danger. Ford knows and is aware of the design flaw and has refused to issue a recall, not only affecting my safety, but now I am going back and forth with ford over the fix to the tune of $2800 that they refuse to fix (vehicle is a gold certified cpo, has extended warranty). When asked how to prevent in the future, ford was and is not aware of how to prevent in the future, and I can not get to the cowel drain clog without professional equipment and ford performing. There is no way to tell the cowel drain is clogged or not, and there is not anything in the ford owners manual related to maintenance of the cowel drain. Again, due to the inappropriately designed drain, this caused water leakage into my truck, posing an immediate threat to my life and others around me. I have all of the videos and pictures needed to show the failure of the system to provide to move this forward to get resolved. I had complete loss of power and electronics on the road. I tried to attach video but it would not let me. I can attach via email when requested.

My truck because of the transmission and it's "powertrain" faults; puts itself in low power mode. I can be driving fine, stop for a red light, to turn a corner or before pulling into oncoming traffic leaving a parking lot etc and without warning, no power. It won't go over 5-10 miles an hour. You have to let off the gas and give gas sometimes more than once to get the truck to finally pick up and go. I have almost been hit countless times because I suddenly couldn't get out of the way of oncoming traffic. The dealer has confirmd to me a few times that this is a transmission issue that is sending my powerboost into low power mode. I believe he said the tech that drove it said it was dangerous; but a known issue. Started a few years ago and doesn't happen all the time. Occurs sporadically and lately more and more frequently.

This report is being made within 1 1/2 hours after the incident. It has not been inspected by a dealer, service center, nor any other agency. I was stopped at a local fast food restaurant waiting to pay for my purchase. The 2021 Ford F-150 was in drive with my foot on the brake. No warning lights or other indication were present. Without warning, the engine revved to a high rpm and the truck shot forward about 15-20 feet before I could react by putting my full weight on the brake pedal. The engine quickly slowed. I then put the vehicle in reverse and backed up to the window and placed the transmission in park. The truck is equipped with adaptive cruise control but it was not active at the time of the incident. Had there been another vehicle in front of me, I would have rear-ended them. I work from home and use the truck a couple of times per week and it is available for inspection most days.

Good Afternoon. The Pro Power Onboard high-voltage electrical system has experienced a permanent malfunction, locking out with a "Ground Fault Detected" warning. The vehicle is a 2021 Ford F-150 Rocky Ridge conversion. This failure stems directly from a defective or incomplete execution of Safety Recall 23S35 (NHTSA Campaign 23V613000).The initial remedy applied by the dealership consisted only of adding protective tape to the rear wiring harness. However, due to the increased travel and ride height of the official Rocky Ridge suspension lift, the wiring harness pulled out of its factory retaining side clip. This caused excessive tension and stretching on the high-voltage lines, completely exposing the bare interior colored wires and leaving them vulnerable to road moisture and friction against the metal frame.The system now triggers a permanent short-to-ground error, creating an imminent risk of an electrical short circuit or vehicle fire under the high-voltage (120V/240V) lines. The Ford dealership and corporate customer care refuse to remedy this recurring safety hazard under the recall warranty because it was previously marked as completed, even though the applied remedy failed to safely secure the harness. My Ford F-150 is subject to Manufacturer Recall Number23S65/NHTSA Recall Number23V896. I asked for the repair multiple times, but the dealership repeatedly told me that the parts were on backorder. The hub bolt on my right rear tire cracked and the truck was making a high pitched sound. I took it to the dealer and they said the bolt was cracked and driving it in that condition is dangerous. The truck is not safe to drive, but the parts remain on backorder indefinitely.

A certified independent inspection company discovered welding on the main chassis of my Ford F-150. The welding is located on the front left frame area. I purchased the vehicle brand new, and it has never been in any accident or collision. This welding on a critical structural part creates a serious safety risk. Frame integrity may be compromised in normal driving or in the event of an impact. I transport my children daily in this vehicle, and I am afraid to drive it due to the possibility of structural failure. The official Ford dealer inspected the vehicle and kept it for approximately three weeks but provided no explanation or repair. The regional Ford office has delayed their technical report for more than five months with no response. The defect has been confirmed and is permanent. No warnings or lights appeared prior to discovering the issue. This is a manufacturing-related structural defect affecting vehicle safety, stability, and integrity. A proper safety resolution is required.

Recall 23S65 has been active since January of 2024. Ford has failed to provide the necessary parts to repair my F150. Through my communications to Ford, they have been made aware of the fact that multiple mechanics have indicated that my truck is in need of immediate repair. Ford’s failure to provide the necessary relief to a recall from January of 2024 renders my $45,000 ($72,000 new) truck basically useless to me. It is not safe to drive any distance, nor should it be towing a camper. Ford personnel are not returning my calls to discuss options to provide relief. We have been patient but now believe Ford is negligent in handling this recall and are unwilling or unable to provide necessary equipment and relief. Speaker system/amplifier created a loud sonic boom type noise while I was driving. Made my ears ring and felt very unsafe to drive afterwards. First time this happened, making an appointment with dealership to get it checked out. No error messages came up on the screen or dash. Still do not feel safe driving the vehicle if this can happen again.

Radio started blasting a boom/ static sound from the rear speakers and subwoofer; extremely loud and has occurred multiple times. It had previously made a similiar sound occasionally at start up which when under warranty was always diagnosed as unable to duplicate but is now occuring while driving and I am out of warranty. Very startling and distracting when it occurs. Has occured multiple times and lasts upto an hour before I gave up an parked my vehicle. Have a special needs minimally verbal disabled child who is very sensitive to noise (rear speakers are always turned off via fade to front) who was very impacted by this loud explosion of chaos/ noise when it occured with him in the car. Ford response: since out of warranty can pay for diagnostic and repairs. Don't expect NHTSA to care as they weren't concerned with my complaint regarding complete loss of brakes (incomplete or bad programming) with this truck but maybe this will let others know they arent alone if they have this issue.

When backing up, the rear camera display shows a gray filter over the rear image on the display cluster and the right side warning sensors indicate a blue trapezoid on the cluster for the right side of the vehicle. The computer system malfunctioned; the front panel warning was "Stop vehicle and check regen. brakes". I put my car in drive and when I hit the gas pedal the car reversed instead of going forward. I got it towed to the dealer at Piazza Hyundai in Limerick PA. The service did not provide me with an explanation about the warning and showed little concerns to the issues, I was given a month time frame for looking at my car. I consider this a safety issue as the car did not respond appropriately to D and R, I suspect a computer failure.

Numerous online automotive forums also document other owners experiencing the identical failure, suggesting this is a widespread issue rather than an isolated incident. The vehicle was inspected by an authorized Ford dealership, which confirmed the EPAS module failure and acknowledged familiarity with the issue. No warning lights, dashboard messages, unusual sounds, or handling changes occurred before the failure. The power steering loss happened suddenly and without any prior symptoms, leaving no opportunity to anticipate or react to the malfunction before it occurred.

When the weather reaches freezing temperatures or below. The drivers side door will not unlock or open. It will not unlock using the Remote unlock, Key Fob, manual Key, unlock button on the door or pulling the handle from the inside. If there were an incident that would require exiting the vehicle quickly or emergency services needed to extricate the driver quickly, this would not be possible since the door can not be opened. This only occurs during cold weather. I live in Wisconsin and the temperatures during winter are consistently below freezing. The dealer will not get me in to address for another month to look at it. The truck was purchased February 2022. First sign of the issue occurred December 2022.

1. Electrical System Failure: While traveling on highway, alternator failed suddenly within 2 minutes of “service soon” and “check electrical system” warnings. Caused complete loss of motive power, electronic parking brake locked up while driving, loss of power steering, all gauges failed, transmission shifted into park while in motion. Battery was damaged (“fried”). Engine continued running for several minutes after stop without ability to shut off ignition. Incident documented by Ohio State Highway Patrol. 2. Seat Belt Issue: Driver-side seat belt interlock sticks in unlocked position, creating risk of failure to lock during a crash. 3. Electronic Parking Brake: Intermittently engages while vehicle is moving downhill to a stop (mostly at or below 5 mph). Parking brake light flashes three times then turns solid while in motion. Ford TSB indicates vehicle not eligible for related recall. 4. Infotainment System: Intermittently freezes, requiring forced reboot. Preceded by audio glitches, laggy dash computerized systems. 5. Transmission: Severe clunk and physical jolt when decelerating from ~30 mph to 8 mph. Also occurs during acceleration, particularly upshifts from 2nd to 3rd and 3rd to 4th gears, more pronounced when turning. 6. 4WD System: Intermittently fails to disengage from 4WD HI to 2WD HI. “Failure to shift” message appears several minutes after command to return to 2WD HI. 7. Front Differential/Axle: Previously, faulty front differential housing fractured due to system malfunction, causing intermittent loss of front driveline engagement in 4WD LO (250-500ms delay then re-engagement under throttle). Ford replaced front differential and axle under warranty after vehicle out of service for 2.5 months. Abbreviations: 4WD HI = 4-Wheel Drive High Range; 2WD HI = 2-Wheel Drive High Range; TSB = Technical Service Bulletin.

UNKNOWN Our F-150 was hit by a driver from a side street. The whole passenger side was damaged, front fender replaced, front door replace, post repaired, back door replaced, back panel of truck replaced, and back wheel. The long step was pushed up and under vehicle. It was replaced as well. The side airbag did not engage. I was thrown around in the passenger seat. I experienced a contusion on my right knee and three days later diagnosed with side impact whiplash. The truck was declared not safe to drive while we were trying to get estimates for repairs. They were concerned that the airbag didn’t go off and the door was not closing correctly even after they tried to tweak it. Our truck was towed off by the insurance company to a secured site until we could get it towed for repairs. We were never told why the airbag didn’t go off.

While driving my 2021 Ford F-150 PowerBoost, the vehicle issued a pre-collision warning followed immediately by a restraint system warning. The airbag then deployed without any collision or impact. There was no crash, obstacle, or reason for deployment. The airbag deployment shattered the windshield and caused interior damage. This appears to be a false activation of the pre-collision system and unintended airbag deployment, creating a serious safety hazard. This could easily result in injury or loss of vehicle control. The vehicle needs to be investigated for a potential defect in the airbag system, sensors, or pre-collision assist system.

Started after McGee ford did a service on my truck Apparently they told ford they repaired electrical recalls which I wasn’t having issues with nor did I know They did anything! They did however erase the recalls so that was 8-2024 shortly after my truck experienced major electrical issues, another dealership told me it was gonna be 6k and 4 weeks to fix I was in awe I left with my truck that had no blinkers no air bags and so on 15 safety codes! I battled this for a year and then my parking break locked up while driving 70 on the highway! That’s the recall megee claimed they fixed I’m not rich single dad 100% of rhe time and I sure my kids everywhere daily, I do research call ford find intact i had a recalll so im thinking it will get fixed! 6 weeks later they not fixed i was told 2 days Now heritage and ford won’t fix it they want megee ford the certified service and dealer to pay for it. It’s winter im a contractor in Vermont they let me borrow a Sentra with summer tires on it. Couldn’t make it off my road. I’m still driving my kids around! I almost reck once a day as the whole system is messed up now! I feel like I’m being bullied and picked on because of my nationality

The contact stated that while her husband was stopped, a 2023 Dodge Ram 1500 reversed at approximately 55 MPH, causing the rear to crash into the front of her husband's front of vehicle. No air bags deployed. The contact's husband and grandfather, who was a passenger in the vehicle, were injured. The grandfather sustained neck and facial injuries, primarily around the neck and chin area. The husband suffered injuries to his hands. Medical attention was required for both occupants. A police report was filed, and the vehicle was towed. The insurance company was pending an inspection to determine if the vehicle was destroyed or not. Confirmed through dealership that the Clock Spring in the steering column needs to be replaced as it is defective. Ford has indicated that my vehicle vin is not a part of the current clock spring recall.

On June 17, 2022 while driving to work on highway in Nee York Stare, I put the adaptive cruise control on so I could adjust my drivers side rear view mirror. The truck suddenly lunged to the left striking a guard rail and overturned several times. Despite being a direct high speed impact to the guard rail and overturning , the steering airbag did not deploy. See police report attached. Due to this , I sustained major internal tissue damage including: a broken sternum, punctured lung bruised aorta, 4 fractures to the vertebrae in both my upper and lower spine and 4 broken ribs in my upper rib cage. I spent a total of 11 nights in the hospital including 4 nights in the ICU. I have not been cleared to return to work and instill under medical care. I understand there was a airbag/steering column recall on other F150s but I strongly suspect that the reveal needs to be expanded to mite vehicles such as mine. All the Doctors that treated me said my injuries were the direct result of the steering wheel airbag not deploying. The state trooper that reined me from my vehicle , as well as one of the witnesses both told me that my steering airbag did not deeply. I also have pictures as well.

The contact had pulled over to the side of the road and noticed that the driver’s side front tire was flat. The contact placed the spare tire on the vehicle and continued driving to his residence. The contact took the vehicle to an independent mechanic who inspected the vehicle and the tire and diagnosed the failure as a split in the middle of the tire. The mechanic replaced all 4 tires on the vehicle. The manufacturer was not made aware of the failure. The failure mileage on the vehicle and tire was approximately 4,000. Vehicle has been repaired.

2021 F150 XLT Powerboost (4x4, 9420 miles). Brakes failed while at a complete stop and truck propelled forward despite brake pedal being being pressed down. I was in a fast food drive through at a complete stop with my foot on the brake pedal and the instrument cluster went crazy, warning lights flashing, malfunction messages for what seemed like everything (believe pre-collision, ABS, traction, hill, and some others- cant recall for sure), warning chimes sounding non stop, the brake pedal dropped towards the floor board, and truck started moving forward despite pressing pedal as far down as I could; this all happened almost instantly. Pumped breaks (not sure if it did anything) but I barley stopped in time from hitting the car in front of me, and I left a far amount of room between us initially. After this, when limping the truck through the drive through (was stuck in the line at this point), it took about 10ft to stop from a 1-2mph roll. After clearing the drive through line, I was limping the truck to open area of parking lot and the truck randomly bucked (like bump starting a manual) and all the instrument cluster warnings disappeared and brake pedal pressure and braking performance returned. Just as how the issue instantly occurred, it also disappeared instantly. Despite all the warnings on the cluster, only ABS Fault and Pre-Collision show on the FordPass app. Prior to to this, there were no issues. The vehicle has yet to be inspected by Ford as the service department is booked out for bit. Vehicle has since been parked as I am fearful a 6k lb vehicle without brakes can end with loss of life. Interesting enough, Ford does not seem to share this concern as their Roadside Assistance Dept says the truck does not have towing coverage while their Customer Care Team says it does but they can't work with each other and end result is I had to prepay $273.06 to Ford Roadside Assistance for the tow to the future service appointment, and maybe Ford might reimburse it.
