Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2018-2021 Tundra vehicles. The headlight electrical circuits may power the high and low beams simultaneously, which could overheat the electrical connector.
Risk: An overheated electrical connector can increase the risk of a fire.
Fix: Dealers will modify the engine wire harness assembly and replace any damaged bulb, bulb connector, and/or headlight assembly as necessary, free of charge. Risk: If the fuel pump fails, the engine can stall while driving, increasing the risk of a crash.
Fix: Toyota will notify owners, and dealers will replace the fuel pump assembly with an improved one, free of charge. Owners of specific models were notified that remedy parts were available starting January 21, 2021. Owners of other models will be notified as remedy parts become available. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2018-2020 Toyota Tundra vehicles equipped with LED headlight assemblies. Due to a wiring error, the front turn signals may not flash brightly enough. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 108, "Lamps, Reflective Devices, and Associated Equipment."
Risk: If the turn signals or hazard lamps do not flash brightly enough, other drivers may fail to see that the turn signal or hazard lamp functions have been activated, increasing the risk of a crash.
Fix: Toyota will notify owners, and dealers will modify the wire harness connected to the front turn signal bulbs free of charge. The recall began August 17, 2020. Owners may contact Toyota customer service at [phone removed]. Toyota's number for this recall is 20TB12/20TA12.

The accessory all-weather floormats may have been counted twice when creating the load carrying capacity modification label, resulting in the capacity modification label incorrectly overstating the additional weight by 10 pounds. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 110, "Tire Selection and Rims."
Risk: Since the incorrect label is not within 1% of the additional weight, the vehicles are not compliant with the FMVSS.

The air bag electronic control unit (ECU) may erroneously detect a fault during the vehicle start-up self check. If this occurs, the ECU may not deploy the airbags as intended, in the event of a crash. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 208, "Occupant Crash Protection," and 214, "Side Impact Protection."
Risk: If the air bags do not deploy as intended, it can increase the risk of injury in a crash.
Fix: Toyota will notify owners, and dealers will update the software for the air bag ECU, free of charge. The recall began October 26, 2018. The Forward Recognition Camera on my 2019 Toyota Tundra has suffered an internal failure. The vehicle was inspected by an authorized Toyota dealership, which diagnosed and documented:"FOUND INTERNAL FAULT IN FORWARD RECOGNITION CAMERA." The failed component is available for inspection. This camera is a critical safety component that supports forward collision warning, automatic emergency braking, and other driver-assistance systems. Because of this internal failure, the vehicle's ability to detect potential hazards may be reduced or disabled, leaving the driver without safety features that were designed to help prevent or mitigate crashes. This failure places not only the occupants of my vehicle at risk but also other motorists, pedestrians, and cyclists sharing the roadway. The condition was reproduced and confirmed by an authorized Toyota dealer. The dealer determined that the camera itself had internally failed and requires replacement. The failure was not attributed to an accident, impact, misuse, modification, or outside damage. It was diagnosed as an internal fault of the camera assembly. Prior to diagnosis, warning messages and safety-system malfunction alerts appeared on the vehicle, indicating that the driver-assistance systems were not functioning properly. These warnings prompted me to seek service. The dealership subsequently confirmed the internal camera failure. I am concerned that this component failed under normal operating conditions in a relatively modern vehicle and that other Toyota Tundra owners may be experiencing the same defect. A safety-critical camera system should not experience an internal failure that disables key crash-avoidance technologies. I am requesting that NHTSA investigate whether this defect is affecting other 2019 Toyota Tundra vehicles and determine whether corrective action, including a recall, is warranted. The dealer diagnosis specifically states:"FOUND INTERNAL FAULT IN FORWARD RECOGNITION CAMERA."

I bought this car in January 2019, and even though it had 210 miles, they sold it to me as new. I think they did something illegal because I have the purchase contract and they didn't put that information there. They put 10 miles and I didn't notice at the time. Also, I have the window sticker and it says the car comes standard with a remote key for entry, and the dealer removed it to make it without one. I'm ashamed I didn't report this sooner. It must be a form of consumer theft, something they only do to protect themselves.

The built-in navigation system in my 2019 Toyota Tundra is dangerously outdated and cannot be updated over-the-air (OTA). Recently, it misrouted me into a remote, unfamiliar area with no cell service and incorrect road data. If I hadn’t had enough fuel, I could have been stranded in potentially life-threatening conditions. Toyota continues to install these systems with outdated maps and no OTA functionality, while presenting them as part of a reliable, intelligent driving experience. This is not a minor tech issue—it is a safety failure. Drivers are led to trust a system that cannot be trusted. I want to be clear: this experience has completely changed how I view Toyota as a brand. What seemed like a small, fixable flaw turned into a moment of genuine danger. That moment told me everything I needed to know about Toyota’s priorities—and Toyota is no longer in my future because of it. I urge NHTSA to investigate this as a potential recall-level issue. A navigation system that actively misleads the driver and offers no timely updates is not just broken—it’s hazardous.

We were driving down an unpaved road at 8 mph in 4WDh in our 2019 Toyota Tundra when I heard a couple popping sounds from inside the car. Then it stopped. I opened the passenger side door to get out and check on the truck, and flames were pouring out under the engine. There were 3 of us in the car. We immediately got out and tried to pour water and throw sand on the fire. It grew very quickly, so we ran away. The car was immediately engulfed in flames like something out of a movie with black smoke going high into the air. Within 5-10 minutes, the truck was totally gone. There were no warning signs/lights other than the 2 pops and then fire/explosion. We are very lucky we did not die. We bought it brand new. It was the first new car we’ve ever owned. It was impeccably cared for. We always brought it to the Toyota dealership in Eugene, OR for service. It was there about a month prior to the explosion. During that visit, they replaced the battery as well as dealt with the halogen light recall. Other things were more routine maintenance. We are in shock and do not want this to happen to others. We also do not know what our next steps are, but we wanted your agency to be aware of this issue. The local dealership said it must have been a car defect or bad workmanship at our last service. Either way, it was terrifying.

The fuel pump and the fuel pump control module system failed. Before the failure, I drove my 2019 Toyota Tundra on XXX from [XXX] to [XXX] with no prior check engine light warning. It was parked for all 3 days at a campground in [XXX]. However, on XXX the vehicle would not start. The truck ignition was moved to the RUN position and then the check engine light came on, which was the first notification of a potential problem. So, I attempted to start the vehicle, there was a crank but not a start incident. The ignition was moved back to the OFF position. I'm also an experienced mechanic, so I lifted the hood to diagnose the issue and search where the malfunction was occurring. I inspected the battery, all the fuses, the relays controlling the fuel pump, and the fuel pump control module. Using a Voltmeter Tester, the battery had a full charge. The relays and the fuses were also replaced with no success in starting the vehicle. The fuel line was disconnected from the fuel rail so I could verify if fuel was getting to the engine. The truck ignition was again moved to the RUN position and then the Start Position with NO success of fuel getting to the fuel rail. The vehicle was then towed to a local auto shop and the problem has been reproduced only by Crawford Auto Repair shop in [XXX]. The auto mechanic mentioned two error codes were presented: 1. The fuel pump 2. The fuel pump control module. Thankfully, there was no immediate risk to my family's safety but it could have occurred while driving back through the [XXX] mountains on our way home. We had just exited the interstate when my Toyota Tundra stopped. We had it towed to the closest Toyota dealer. We had to have fuel pump replaced. They said the previous year Tundra was recalled for this issue, but ours was not. I just found out that some of the 2019 models had been recalled for this issue. They said my VIN was not recalled. That does not make sense. It was the same problem, same year, just at a different plant. Now we are having other fuel issues and the dealer says it all stems from the replacement of the pump, which we had to pay out of pocket. If we had not just exited the interstate it could have caused a major accident.

My toyota tundra is having issues with the fuel pump. It is saying I am using e85 fuel when that is not the case. As a result I am having issues with cold start. There are recalls however I am being told that my truck is listed among them despite having the same issue.

Having fuel pump issue. Car stalls very badly and pre collision light and engine lights start flashing. Severe jumping by car driving down the road. When accelerating the car has no acceleration and hesitates very badly.

The contact owns a 2019 Toyota Tundra. The contact stated that while parked for 5-10 minutes, the contact noticed a burning plastic odor, and upon looking up moments later, the contact noticed flames coming from the driver's side headlight, and the flames were 6 inches above the headlight. The contact started extinguishing the fire with water bottles. The contact drove to a water hose, which the contact also used to extinguish the fire. The contact sustained minor smoke inhalation. No police report was filed. The contact was able to drive to the nearby residence. The contact related the failure to NHTSA Campaign Number: 21V688000(Exterior Lighting, Electrical System), which included the VIN. The manufacturer was notified of the failure and sent a field inspector, who inspected the vehicle and informed the contact that the inspection report would be completed on the same day; however, the manufacturer would follow up with the contact within 45-60 days. The contact followed up with the manufacturer three days later and was provided with the same information. The dealer was not contacted. The vehicle was not diagnosed or repaired. The failure mileage was approximately 100,500.

Intermittent message displaying "Trailer Brakes Disconnected" while towing. Trailer lights continued to operate even while message was displayed. On average trailer connected and disconnected every 2-3 minutes

Keep getting the messages "Trailer brake error, if towing come to a stop with manual controls" And "Pre-collision system malfunction" The messages go away usually after restarting the vehicle but eventually comes back on

The contact owns a 2019 Toyota Tundra. The contact stated while driving at an undisclosed speed on several occasions, the vehicle stalled. There was no warning light illuminated. The vehicle failed to restart and was towed to the dealer on two occasions. The dealer diagnosed that the fuel pump had failed and needed to be replaced; however, the fuel pump part #770200C170 was on backorder. The dealer reprogrammed the fuel pump system twice as temporary repairs; however, the failure persisted. The contact stated while driving 35 MPH, the vehicle stalled. There was no warning light illuminated. The contact was unable to pull over to the side of the road. The vehicle failed to restart and was towed to the residence, where it was diagnosed by a mobile mechanic. The mechanic diagnosed that that the fuel pump was burned, causing the electrical connector part #[phone removed] to burn and melt. The dealer was contacted and informed the contact that the electrical connector part had been discontinued. The vehicle was not repaired. The manufacturer was notified of the failure and informed the contact that the vehicle was not covered under the factory warranty. The failure mileage was approximately 102,000.

The electrical system malfunctioned, causing all warning lamps on the dashboard to light up. Because this happened while I was driving, I do believe that this electrical issue caused safety concerns for others on the road. The taillights went out first, seconds later, all remaining electrical failed at once, including airbags, windows, headlights at one point, pre collision warning lights, lane departure, and the breaking system failed as well. This caused safety concerns for myself, my passenger, as well as the public because all safety warning sensors failed at once while on the road. The problem has been confirmed by Roseville Toyota, who is a manufacturer, and who currently has the vehicle now, they said that there is definitely an electrical issue and most likely a short of some sort. The warning lamps and messages on the dash lit up one at a time, seconds apart from each other, until the entire dash was lit up completely while I was driving on March 24, 2023.

The Forward Recognition Camera on my 2019 Toyota Tundra has suffered an internal failure. The vehicle was inspected by an authorized Toyota dealership, which diagnosed and documented:"FOUND INTERNAL FAULT IN FORWARD RECOGNITION CAMERA." The failed component is available for inspection. This camera is a critical safety component that supports forward collision warning, automatic emergency braking, and other driver-assistance systems. Because of this internal failure, the vehicle's ability to detect potential hazards may be reduced or disabled, leaving the driver without safety features that were designed to help prevent or mitigate crashes. This failure places not only the occupants of my vehicle at risk but also other motorists, pedestrians, and cyclists sharing the roadway. The condition was reproduced and confirmed by an authorized Toyota dealer. The dealer determined that the camera itself had internally failed and requires replacement. The failure was not attributed to an accident, impact, misuse, modification, or outside damage. It was diagnosed as an internal fault of the camera assembly. Prior to diagnosis, warning messages and safety-system malfunction alerts appeared on the vehicle, indicating that the driver-assistance systems were not functioning properly. These warnings prompted me to seek service. The dealership subsequently confirmed the internal camera failure. I am concerned that this component failed under normal operating conditions in a relatively modern vehicle and that other Toyota Tundra owners may be experiencing the same defect. A safety-critical camera system should not experience an internal failure that disables key crash-avoidance technologies. I am requesting that NHTSA investigate whether this defect is affecting other 2019 Toyota Tundra vehicles and determine whether corrective action, including a recall, is warranted. The dealer diagnosis specifically states:"FOUND INTERNAL FAULT IN FORWARD RECOGNITION CAMERA."

Keep getting the messages "Trailer brake error, if towing come to a stop with manual controls" And "Pre-collision system malfunction" The messages go away usually after restarting the vehicle but eventually comes back on

Having fuel pump issue. Car stalls very badly and pre collision light and engine lights start flashing. Severe jumping by car driving down the road. When accelerating the car has no acceleration and hesitates very badly.

This is the second chrome hood bulge that I've put on it and it keep breaking with the wind and just flys off. Also the collision light will come on by itself.

The contact stated that while parked for 5-10 minutes, the contact noticed a burning plastic odor, and upon looking up moments later, the contact noticed flames coming from the driver's side headlight, and the flames were 6 inches above the headlight. The contact started extinguishing the fire with water bottles. The contact drove to a water hose, which the contact also used to extinguish the fire. The contact sustained minor smoke inhalation. THE #4157 INCANDESCENT TURN SIGNAL BULBS ON THE FRONT OF THE TRUCK ARE FAR TOO DIM AND GET DROWNED OUT BY THE LED DAYTIME RUNNING LIGHTS AND LED HEADLIGHTS. Sunroof shattered while driving on highway. Noise similar to a gun being fired, caused momentary disorientation; however, was able to keep vehicle on the road. No warning, no other vehicles around that could have kicked up a rock, debris, etc. Smooth conditions on the road (no potholes, bumps, etc.). Sunroof was in the tilt up position, side windows down, and rear window down - even temperature outside and inside (74 degrees F). Small glass "shards" in eyes and hair; however, most of it went up/out. Vehicle gets inspected tomorrow (09-21) for insurance purposes.

I took my vehicle in for scheduled maintenance. During the safety inspection the Toyota authorized tech advised me about a serious safety issue with rust on the undercarriage of my vehicle. He advised that the control arms as well as sway bars and suspension , shocks, struts had heavy rust on them. He advised that this is a safety issue as if something breaks I could lose control. I have enclosed the video. I then contacted First Team Toyota and they initially wanted me to bring the truck in for inspection. The next day they called and told me that since I was no longer under warranty , That they could not help me. I was also told that the undercoating I paid extra for does not cover the parts that are rusted. I contacted Toyota corporate and they opened a case #[XXX]. After several days they decided that there was nothing they could do for me. I have my vehicle serviced routinely and have never been advised of any rust problems on my truck. Then after the warranty runs out, I’m advised of this problem. I take extreme care of my truck and spray the under carriage with fresh water whenever I drive thru rain or puddles. My son was driving to Mineral Wells to get something to eat on Wednesday 11/17/21 at 8:00 at night on FM 52 South. All of a sudden the steering wheel falls in his lap. He picked it up and shoved it onto the column and was able to steer the truck onto the side of the road without hitting anything. He looked underneath the steering wheel assembly and 2 of the 4 ubolts where missing. Where did they go - if they were ever there in the first place. The steering wheel has had some vibration and we have had the truck front end aligned and the wheels balanced. I had it in not long ago for an inspection/oil change/tire rotation. After I left the dealership the wheel felt a little loose and there was some vibration but I thought it was the road and it did stop vibrating. I also drive a Prius and I thought maybe I'm just not used to the way the truck feels. I told my son when I got home that it felt loose and he said he would keep an eye on it. Then Wednesday night the steering wheel falls off and the whole column falls out. What is this? I also drive the truck and I do not think I would have had #1 - the strength to pick the wheel up and force it back onto the column or #2 - the sense to even do that. I would have had an accident. The Shottenkirk dealership does not want to deal with the problem even though I have an extended warranty so today Freeman Toyota calls me and they are picking up the truck and will be doing the repair work. Bottom line - my son could have been killed or injured, I drove the truck Monday in Ft. Worth going 70+mph and I could have been killed or injured. How does a steering wheel/steering column loose ubolts and falls off/collapses?

I WAS DRIVING DOWN THE ROAD ON THE FREEWAY WHERE THE SPEED LIMIT WAS 75 MPH. WHILE DRIVING MY PASSENGER SAID I PASSED OUT, FLOORED THE GAS PEDAL AND JERKED THE STEERING WHEEL TO THE LEFT. I THEN DROVE THROUGH THE MEDIAN AND INTO ONCOMING TRAFFIC REACHING 90 PLUS MPH, MISSING TWO CARS, AND JUMPING A 4 FOOT BARBED WIRE FENCE WITHOUT TOUCHING THE FENCE. I CONTINUED INTO THE DESERT UNTIL MY PASSENGER WAS ABLE TO PUT THE TRUCK IN PARK HITTING A FEW TREES AND BUSHES ALONG THE WAY. I THEN WOKE UP WHEN PARAMEDICS AND FIRST RESPONDERS WERE ALREADY THERE. ME AND THE PASSENGER WERE BOTH WEARING OUR SEATBELTS AND MY PASSENGER DID NOT AQUIRE ANY INJURIES. I HAD A SIEZURE TO CAUSE THE ACCIDENT AND HAVE HAD MULTIPLE ONES SINCE THEN A MONTH AGO. THE FRONT OF MY TRUCK WAS MANGLED AND THE FRAME WAS BENT IN TWO PLACES. THE ISSUE IS I SUFFERED 4 FRACTURES IN MY SPINE TOP OF THE LUMBAR AND BOTTOM 3 OF MY THORASIC. I AM CURIOUS AFTER TALKING TO THE MECHANIC WHO LOOKED OVER MY TRUCK AS TO WHY MY AIRBAGS DID NOT DEPLOY DURING THE ACCIDENT. IN SUMMARY I WAS IN AN ACCIDENT THAT TOTALED MY TRUCK, AND THE AIRBAGS DID NOT DEPLOY AND I SUFFERED MULTIPLE SPINAL FRACTURES.

3 times in the last few mos the BRAKE! warning came on. Nothing was causing this warning to engage. This happened again today. Another issue is when the Valdosta Toyota (GA) dealer attempted to fix the fuel pump recall on 22 JUL 2020 it was assembled incorrectly. I called the dealership and told them to come and get my Tundra because gas was dripping onto my driveway. They sent a tow truck and it was supposedly fixed and they brought it back to me in about 2 hrs. It really does run the way it did before they “re-fixed” it. Very often now when it is driving I feel something like a failing fuel pump. It fades away but it is not comfortable taking it long distances. It feels like there is a clog in the fuel system or something, although there is not. I have 33,400 miles on it today and I can’t help but wonder if Valdosta Toyota didn’t just exchange the original fuel pump with a different used one?

I was on a camping trip in Wyoming, and I started my Tundra up on a chilly morning. It was running for about 5 minutes before I came back to it and entered the cab. Immediately upon entering I noticed a strange smell. I also noticed at that time that the driver’s seat was extremely warm. I quickly shut off the heat warmer (original equipment on the truck). Inspecting the seat, I could see where the leather was just starting to melt. Luckily, I caught this in time, for if I would have left it longer, the vehicle could have caught fire. An online search discovered previous problems with Toyota seat warmers. My contact with Toyota was not helpful, for they simply told me that it was out of warranty, despite being a safety issue.

The seat heater control module has failed causing the plastic to burn and melt, nearly starting a fire. Toyota knows about this but refuses to issue a safety recall.

I took my vehicle in for scheduled maintenance. During the safety inspection the Toyota authorized tech advised me about a serious safety issue with rust on the undercarriage of my vehicle. He advised that the control arms as well as sway bars and suspension , shocks, struts had heavy rust on them. He advised that this is a safety issue as if something breaks I could lose control. I have enclosed the video. I then contacted First Team Toyota and they initially wanted me to bring the truck in for inspection. The next day they called and told me that since I was no longer under warranty , That they could not help me. I was also told that the undercoating I paid extra for does not cover the parts that are rusted. I contacted Toyota corporate and they opened a case #[XXX]. After several days they decided that there was nothing they could do for me. I have my vehicle serviced routinely and have never been advised of any rust problems on my truck. Then after the warranty runs out, I’m advised of this problem. I take extreme care of my truck and spray the under carriage with fresh water whenever I drive thru rain or puddles. REAR AXLES AREN'T THE PROPER FIT, CAUSING THEM TO SHAKE WHICH CAUSES THE ROTORS TO VIBRATE THAT PRODUCES AN SQUEAKY/STRETCHING NOISE WHEN BRAKING. WAS ORIGINALLY TOLD IT WAS NORMAL UNTIL I CONTACTED TOYOTA CUSTOMER CARE. SERVICE TECHS STATES THAT 'IT'S SAFE TO DRIVE' BUT I DIDN'T TRUST IT. MY TRUCK ISN'T THE ONLY TRUCK, THERE ARE SERVAL MORE THAT MAKING THIS SAME EXACT NOISE. ABOUT TO TURN MY TRUCK BACK IN TO TOYOTA UNDER THE LEMON LAW.

TL* THE CONTACT OWNS A 2019 TOYOTA TUNDRA. WHILE DRIVING APPROXIMATELY 70 MPH, AN EXPLOSION WAS HEARD. THE VEHICLE WAS PULLED OVER AND THE CONTACT NOTICED THAT THE SUN ROOF EXPLODED WITHOUT AN IMPACT OR WARNING. THERE WERE NO INJURIES. THE CONTACT CALLED OAKBROOK TOYOTA AT [phone removed] (LOCATED AT [address removed]) AND WAS INFORMED THAT THE FAILURE HAD TO BE CAUSED BY SOMETHING STRIKING THE SUN ROOF. WHILE DRIVING 40 MPH THE WARNING LIGHT 'ACCELERATOR AND BRAKE PEDALS DEPRESSED SIMULTANEOUSLY ' CAME ON AND THE TRUCK SLOWED DOWN SIGNIFICANTLY, WHICH ALMOST CAUSED AN ACCIDENT. THE BRAKE PEDAL WAS NOT BEING USED WHILE DRIVING.

WHILE DRIVING WITH THE ADAPTIVE CRUISE CONTROL SET A MESSAGE WILL APPEAR "RADAR CRUISE CONTROL UNAVAILABLE CLEAN SENSOR". THIS MESSAGE WILL APPEAR WITH NO ADVERSE WEATHER CONDITIONS. DUE TO THIS ERROR THE CRUISE CONTROL NO LONGER WORKS AND THE PRE-COLLISION WARNING SYSTEM NO LONGER WORKS. ALSO RANDOMLY A "BRAKE!" MESSAGE WILL APPEAR AND THE VEHICLE AUTOMATICALLY SLAMS ON THE BRAKES WHEN NO EMERGENCY EXISTS CREATING A HAZARD FOR VEHICLES BEHIND.

The contact owns a 2019 Toyota Tundra. The contact stated that while replacing the shocks, the contact noticed that the shocks were oily. The vehicle was taken to the local dealer for warranty repair, and the contact was informed that the brake pads on the rear axle were soaked with oil. The vehicle was not repaired. The manufacturer was not contacted. The failure mileage was approximately 30,000.

Toyota Safety Sense (TSS) malfunctions sporadically while driving. Warning lights illuminate and Dynamic Radar Cruise Control (DRCC) disengages when activated while driving. The Pre-Collision System (PCS) is turned off and flashes with the following error messages. - Pre-Collision System Malfunction - Visit Your Dealer - Auto Headlight System Malfunction - Visit Your Dealer - LDA Malfunction - Visit Your Dealer This has happened twice, first right after purchasing the vehicle brand new in 2019 and most recently last week on 5/22/2021. I took the vehicle into the dealer on 5/25/2021 and they seemed aware of this issue, but stated they could not reproduce it and there were no error codes logged. I showed them the video of this happening (link below) while driving through Nevada. I was told the errors will not disable the vehicle or put it into limp mode, but my concern is when this happens while using DRCC, which almost made me drive into the back of a tractor trailer on Interstate 80. VEHICLE PARKED ON BOAT LAUNCHING RAMP 25-30% SLOPE CONCRETE SURFACE WITH 24 FOOT ALUMINUM TRAILER IN TOW; BOAT OFF TRAILER, TRUCK FACING AWAY FROM WATER, TRAILER CLOSEST TO WATER. SUNNY, RAMP WAS DAMP, WITH SOME WOOD AND AQUATIC VEGETATION ON RAMP. VEHICLE WAS IN 2 WHEEL DRIVE. AFTER UNLOADING BOAT USING A LAUNCH CORD AND PULLING BOAT ONTO RAMP, DEPRESSED BRAKE AND MOVED SHIFTER FROM DRIVE TO PARK. PARK ENGAGED AND STEPPED FROM VEHICLE WHEN TRUCK BEGAN TO ROLL BACKWARD. LUNGED INTO CAB, SAW SHIFTER WAS NOW IN NEUTRAL! WAS ABLE TO REACH SHIFTER AND ATTEMPTED TO PUT INTO PARK. GEAR SHIFT MOVED TO PARK BUT VEHICLE CONTINUED TO ROLL AND I HEARD AN AUDIBLE CLICKING FROM TRANSMISSION AREA. TRUCK (AND ME) CONTINUED BACKWARD INTO LAKE, ALMOST DROWNED BUT FOR HELP OF A GOOD SAMARITAN ON SHORE. WHEN TRUCK RECOVERED IT WAS IN PARK, WRECKER OPERATOR HAD TO MANUALLY BYPASS THIS TO LOAD ON FLATBED. MY PRIOR 2019 TUNDRA CREW CAB DID SAME THING I CANNOT BELIEVE THIS HAPPENED. IN 2007 ON YOUR SITE SOMEONE REPORTED SAME TUNDRA ISSUE UNDER CONTROLLED CONDITIONS. SOMETHING IS WRONG WITH THIS TRUCK'S TRANSMISSION.
